REGISTRATION

All persons interested in participation must be registered via an on-line registration form no later than 4 December 2019

REGISTRATION FEE

Students, nurses and doctors from Department of Neurology FNOL do not pay the registration fee.

Registration fee will be paid via bank transfer and includes organizational costs, refreshments, lunch, dinner, one printed copy of the conference proceedings. Payment should be made by 9 December 2019; after this date please make the payment of the conference fee in cash or by card at the registration desk.

Number of the rooms is limited and the offer is applicable only till 12 December 2019. The accommodation is paid at the hotel reception desk.

Accommodation for the invited speakers is provided by Conference Centre of Palacký University.

MEALS

There will be two coffee breaks, conference dinner on 12 December and lunch on 13 December provided. The conference dinner will take place at Café Restaurant Caesar (Horní náměstí 583, Olomouc). Please mark your conference dinner participation in the registration form.

CANCELLATION OF PARTICIPATION

In case of cancellation, please contact the Conference Centre at romana.sedlackova@upol.cz. The cancellation deadline for full registration fee refund is 10 December 2019. Refunds will not be considered for requests received after this deadline date. All approved refunds will be issued after the Conference.
